Introduction & Scope
These Terms of Use (“Terms” or “Agreement”) govern your access to and use of the MatchMyDentist platform, services, and website, operated by MatchMyDentist Inc., a Delaware corporation with its principal place of business at 8 The Green, STE A, Dover, DE 19901 (“MatchMyDentist,” “Company,” “we,” “us,” or “our”).
MatchMyDentist is a proprietary, AI-enabled technology platform that enables prospective dental patients (“Patients”) to create anonymous Patient Profiles describing their dental needs, goals, preferences, and other information they choose to share. Participating independently licensed dentists (“Providers”) may review eligible Patient Profiles and, at their sole discretion, send personalized introductions through the Platform. If a Patient independently accepts a Provider’s introduction, MatchMyDentist facilitates the exchange of contact information so the Patient and Provider may communicate directly.
MatchMyDentist is not a healthcare provider, does not provide dental care or professional advice, does not recommend, endorse, or refer Patients to any Provider, and does not participate in the clinical relationship between Patients and Providers. All decisions regarding whether to send, accept, or decline an introduction are made independently by the Patient and the Provider. The Services are provided at no cost to Patients.
By accessing or using the platform (the “Platform”), you agree to be bound by these Terms. If you do not agree, you may not use the Services.
These Terms incorporate by reference the following supplemental policies (collectively, the “Supplemental Policies”): the Privacy Policy and the Consumer Health Data Privacy Policy.
The Services are intended for individual, personal use.
Definitions
“MatchMyDentist,” “Company,” “we,” “us,” or “our” — MatchMyDentist Inc., a Delaware corporation that owns and operates the Platform.
“Platform” — the proprietary, AI-enabled technology platform that enables Patients to create anonymous Patient Profiles, enables Providers to review eligible Patient Profiles, facilitates personalized introductions initiated by Providers, and, upon mutual acceptance, facilitates the exchange of contact information between Patients and Providers. The Platform is not a clinical service.
“Services” — the features, tools, systems, communications, software, and infrastructure offered through the Platform, including Patient Profile creation, Provider profile management, Patient Profile review, introduction tools, messaging functionality, and related infrastructure.
“User,” “you,” or “your” — any individual who accesses or uses the Platform.
“Patient” — any person seeking dental care who creates a Patient Profile through the Platform.
“Provider” — an independently licensed dentist or dental practice participating in the Platform.
“Patient Profile” — the anonymous profile created by a Patient containing information such as dental concerns, goals, treatment interests, budget, insurance information, preferences, and other information the Patient chooses to provide. Patient Profiles are used solely to facilitate potential introductions between Patients and Providers.
“Introduction” — a personalized message initiated by a Provider through the Platform expressing interest in connecting with a Patient. An Introduction does not create a Provider-Patient relationship and does not guarantee that the Patient will respond, accept, schedule an appointment, or receive treatment.
“Match” — a connection that occurs only after both the Patient and the Provider independently agree to connect through the Platform. Following a Match, MatchMyDentist facilitates the exchange of contact information. A Match does not constitute a referral, endorsement, recommendation, guarantee of treatment, or medical advice.
“Content” — any data, text, images, messages, documents, information, or other materials submitted, transmitted, or made available through the Platform.
Description of Services
3.1 Patient Profiles and Provider Review
A Patient completes a confidential Patient Profile that may include information such as treatment goals, dental history, location, budget, insurance status, availability, dental concerns, and other information the Patient chooses to provide. The Platform uses this information to make eligible Patient Profiles available to participating Providers based on Platform criteria and Provider participation settings. Patients do not browse or search participating Providers, and Providers do not pay for visibility, advertising, rankings, or placement within the Platform.
3.2 Anonymous Patient Profiles, Provider Introductions, and Mutual Acceptance
Providers review anonymous Patient Profiles without access to the Patient’s name or contact information. If a Provider chooses to connect, the Provider may submit a personalized introduction through the Platform for the Patient to review. The Provider’s identity is disclosed to the Patient together with the Provider’s introduction. The Patient’s identity and contact information remain confidential unless and until the Patient independently accepts the Provider’s introduction. If both the Patient and the Provider independently agree to connect, MatchMyDentist facilitates the exchange of contact information, and a Match is established.
3.3 No Clinical Role
MatchMyDentist is a technology platform and does not provide dental services, clinical advice, diagnoses, treatment recommendations, or healthcare services. MatchMyDentist is not a dental practice, healthcare provider, insurance company, or referral agency and does not direct, supervise, or control the professional judgment of any Provider. All clinical decisions, communications, examinations, diagnoses, treatment recommendations, treatment, billing, and follow-up care are managed solely between the Patient and the Provider after a Match has been established.
3.4 No Cost to Patients
The Platform is provided at no cost to Patients. Patients are not charged to create a Patient Profile, receive or review Provider introductions, accept or decline an introduction, or use the Platform’s matching services.
3.5 No Guarantee of Match or Outcome
MatchMyDentist does not guarantee that a Patient Profile will receive a Provider introduction, that a Provider will submit an introduction, that a Patient will accept an introduction, that a Match will occur, or that any Match will result in an appointment, treatment, or any particular clinical or financial outcome. All interactions, communications, appointments, and treatment decisions following a Match occur independently of the Platform.
Eligibility & Account Use
4.1 Eligibility Requirements
You must be at least 18 years old to use the Platform. By using the Services, you represent and warrant that you are 18 or older. The Platform is available only to individuals located in the United States, and is intended solely for private-pay or privately insured Patients. You may not use the Platform if you are enrolled in Medicare, Medicaid, Tricare, CHAMPVA, or any other government-funded health program. A pre-screening step requires you to confirm these eligibility points before you proceed; answers indicating ineligibility will end the process. We reserve the right to restrict access based on geographic location, eligibility, or regulatory limitations.
4.2 Account Registration
You agree to provide accurate, complete, and current information, and to keep it current. You are responsible for the confidentiality of your login credentials and for activity under your account, and must notify us at support@matchmydentist.com if you believe your account has been compromised.
4.3 Personal Use Only
The Platform is for individual, personal use. You may not use the Services on behalf of another person or organization without our express written permission. Commercial, group, or resale use is prohibited unless authorized.
4.4 Suspension or Termination
We may suspend or terminate access for violation of these Terms, prolonged inactivity, false or misleading information, unauthorized use, or to comply with law.
User Conduct & Responsibilities
5.1 Provide Accurate Information
You agree to provide truthful, complete, and current information. The quality of your match depends on it. MatchMyDentist does not independently verify Patient data; you are responsible for consequences of false or incomplete information.
5.2 Lawful and Appropriate Use
You agree to use the Platform only for lawful purposes. You may not impersonate any person, misrepresent your identity, age, insurance status, or dental history, submit fraudulent information, interfere with the Platform, attempt unauthorized access, use the Platform to solicit or serve government-insured patients, or collect information about others without authorization.
5.3 Respectful Communication
You agree to communicate respectfully with Providers. We do not monitor message content but may revoke access for abusive or harassing conduct.
5.4 No Commercial or Unauthorized Use
You may not resell, sublicense, or use the Platform for commercial gain, or to coordinate multiple patients as a broker or agency, without our written permission.
Provider Matching & Engagement
MatchMyDentist facilitates potential connections between Patients and Providers through a closed, proprietary technology platform. The Platform does not operate as a public directory and does not permit Patients to browse, search for, or select participating Providers.
6.1 Patient Profile Submission
Patients create a confidential Patient Profile that may include information such as dental concerns, treatment goals, budget, insurance status, availability, urgency, location, provider preferences, and any other information the Patient chooses to provide. Patients are solely responsible for ensuring the information they submit is accurate, complete, and current.
6.2 Algorithmic Match Generation
Using proprietary algorithms and artificial intelligence, the Platform analyzes information contained in each Patient Profile together with information contained in participating Provider Profiles. Through sophisticated proprietary processing, the Platform identifies the participating Provider or Providers whose practice characteristics, participation criteria, and other proprietary Platform factors most closely correspond to the information provided by the Patient. Those Provider(s) are then presented with the anonymous Patient Profile for independent review.
Providers do not pay for visibility, advertising, rankings, placement, priority, or preferential selection. MatchMyDentist does not recommend, endorse, rank, or guarantee that any Provider is the most appropriate or best Provider for any Patient. The Platform’s algorithms, selection criteria, and processing methodologies are proprietary and may be modified or refined at any time.
6.3 Provider Review and Introductions
Providers review anonymous Patient Profiles without access to the Patient’s identity or contact information. A Provider who wishes to connect with a Patient may submit a personalized introduction through the Platform. MatchMyDentist may offer an AI-generated draft introduction as an optional writing aid; however, the Provider is solely responsible for reviewing, editing, approving, and submitting any introduction sent through the Platform. MatchMyDentist does not author, edit, verify, or endorse the content of Provider introductions.
6.4 Match Completion
Patients independently decide whether to accept or decline a Provider’s introduction. If the Patient accepts the introduction, and both parties independently agree to connect, MatchMyDentist facilitates the exchange of contact information between the Patient and the Provider. Upon completion of that exchange, a Match is established, and any further communication or relationship occurs independently of the Platform.
6.5 No Guarantees
Participation in the Platform is voluntary. Patients are under no obligation to accept any Provider’s introduction, and Providers are under no obligation to submit an introduction in response to any Patient Profile. MatchMyDentist does not guarantee that any Patient Profile will receive a Provider introduction, that any introduction will be accepted, that a Match will occur, or that any Match will result in an appointment, treatment, or any particular outcome.
Communications & Notifications
7.1 Transactional Communications
By using the Platform, you agree to receive communications necessary to facilitate your use of the Services, including notifications regarding your Patient Profile, Provider introductions, Match status, confirmation of Match completion, and optional follow-up communications. These communications are primarily transactional in nature. MatchMyDentist does not send promotional or marketing communications except for optional service updates that you have affirmatively elected to receive, consistent with our Privacy Policy.
7.2 No Phone or Text from MatchMyDentist
MatchMyDentist does not contact Patients by telephone, SMS, text message, or messaging applications regarding their Patient Profile or Provider introductions, and will never request passwords, payment information, or other sensitive information by phone or text. Following a Match, a participating Provider may contact you directly using the contact information you chose to share through the Platform.
7.3 Responsibility for Contact Information
You are responsible for maintaining a current and accurate email address and other contact information associated with your account so that you may receive Platform communications.
No Cost to Patients
8.1 Free for Patients
The MatchMyDentist Platform is provided at no cost to Patients. Patients are not charged to create a Patient Profile, receive or review Provider introductions, accept or decline a Provider introduction, establish a Match, or otherwise use the Platform.
8.2 No Patient Liability
Patients are never charged any fee for using the Platform. Any compensation paid to MatchMyDentist by participating Providers is solely for participation in and access to the Platform and is never contingent upon, based upon, or calculated according to whether a Patient receives treatment, schedules an appointment, or incurs any cost of care. No fees paid to MatchMyDentist are added to a Patient’s cost of dental treatment.
No Clinical Role, No Referral Relationship, and No Dentist-Patient Relationship
9.1 No Dental Advice or Diagnosis
MatchMyDentist does not provide dental services, clinical opinions, diagnoses, treatment recommendations, treatment plans, appointment scheduling, emergency support, or other healthcare services. All care decisions, including whether to establish a dentist-patient relationship, are made independently between the Patient and the Provider.
9.2 No Dentist-Patient Relationship
Use of the Platform, creation of a Patient Profile, receipt or review of a Provider introduction, or communication through the Platform does not create a dentist-patient relationship between you and MatchMyDentist, or between you and any Provider. A dentist-patient relationship, if any, is established solely by the Provider in accordance with applicable law and the Provider’s own policies, and not through use of the Platform.
9.3 No Endorsement or Recommendation
Patient Profiles are presented to participating Providers using MatchMyDentist’s proprietary algorithms and selection methodologies. The presentation of a Patient Profile to a Provider, the submission of a Provider introduction, or the occurrence of a Match does not constitute or imply an endorsement, recommendation, certification, guarantee, or representation regarding any Provider, Patient, treatment, or clinical outcome. Algorithmic selection, Patient Profile presentation, and Provider participation should not be interpreted as a prediction of treatment success, patient satisfaction, clinical appropriateness, or the suitability of any Provider for any Patient.
9.4 Independent Provider Responsibility
Each Provider is an independent healthcare professional solely responsible for their professional judgment, communications, examinations, diagnoses, treatment recommendations, treatment, billing, compliance with applicable laws, and adherence to professional standards. MatchMyDentist does not supervise, direct, or control the clinical practice of any Provider.
9.5 No Duty to Monitor; Right to Remove
MatchMyDentist does not monitor Provider conduct or verify ongoing compliance with professional licensing or legal requirements. However, MatchMyDentist reserves the right, but not the obligation, to suspend, restrict, or remove any Provider from the Platform at any time, including upon notice of licensing issues, criminal activity, violations of these Terms, or patient complaints.
9.6 No Appointment Management or Billing
MatchMyDentist does not schedule appointments, process payments, verify insurance coverage, coordinate treatment, submit insurance claims, or provide billing services. After a Match has been established, all communications, scheduling, financial arrangements, and treatment occur independently between the Patient and the Provider.
9.7 No Browsing or Search
Patients do not browse, search for, compare, rank, or independently select participating Providers prior to receiving a Provider introduction. The Platform does not operate as a public directory and does not provide ratings, rankings, reviews, or searchable Provider listings.
9.8 No Responsibility for Patient-Provider Agreements
MatchMyDentist is not a party to any appointment, treatment agreement, financial arrangement, informed consent, payment obligation, insurance claim, or other agreement between a Patient and a Provider. Any such agreements are solely between the Patient and the Provider, and MatchMyDentist bears no responsibility or liability for their formation, terms, performance, enforcement, or breach.
Privacy, Anonymity & Health Information
MatchMyDentist is committed to protecting your information throughout your use of the Platform and throughout the Patient Profile, introduction, and Match process.
10.1 Identity Stays Private Until You Accept
Before you accept a Provider’s introduction, participating Providers receive only an anonymous Patient Profile, which may include information such as your dental concerns and goals, budget, insurance status, availability, general location, preferences, and other information you choose to provide. Your identity remains confidential unless and until you independently accept a Provider’s introduction. Your name and contact information are securely maintained by MatchMyDentist solely to facilitate the Platform’s services and are not disclosed to a Provider until a Match has been established.
10.2 Post-Match Contact
If you accept a Provider’s introduction and a Match is established, the selected Provider receives your contact information, and subsequent communications occur directly between you and the Provider outside the Platform.
10.3 Use of Your Data
Information you submit is collected, used, stored, and disclosed in accordance with our Privacy Policy and Consumer Health Data Privacy Policy. MatchMyDentist does not sell your personal information and does not use your health-related information for advertising or third-party marketing purposes. You may request access to, correction of, or deletion of your information by contacting support@matchmydentist.com.
10.4 Service Providers
MatchMyDentist uses carefully selected third-party service providers, including providers of secure hosting, cloud infrastructure, identity verification, communications, analytics, and email delivery services, to operate the Platform. These service providers are contractually obligated to maintain appropriate confidentiality and security safeguards and may access only the information reasonably necessary to perform services on our behalf, as further described in our Privacy Policy.
10.5 Health-Data Consent
Some information you provide relates to your dental health, treatment interests, insurance information, and other health-related concerns. By creating a Patient Profile, you consent to MatchMyDentist collecting, storing, processing, and using that information to operate the Platform, generate Provider selections through our proprietary algorithms, present your anonymous Patient Profile to eligible participating Providers, facilitate Provider introductions, and, upon your independent acceptance of a Provider’s introduction, disclose your contact information and relevant Patient Profile information to the Provider with whom you establish a Match. You may withdraw your consent or request deletion of your information as described in our Privacy Policy and Consumer Health Data Privacy Policy.
Disclaimer of Warranties
11.1 “As Is” and “As Available”
The Platform and all Services are provided on an “AS IS” and “AS AVAILABLE” basis, without warranties of any kind, whether express, implied, or statutory. To the fullest extent permitted by applicable law, MatchMyDentist disclaims all warranties, including but not limited to implied warranties of merchantability, fitness for a particular purpose, title, and non-infringement.
11.2 No Warranty of Uninterrupted or Error-Free Operation
MatchMyDentist does not warrant that the Platform will be uninterrupted, timely, secure, or error-free, that the results obtained from the Platform will be accurate or reliable, that any defects or errors will be corrected, or that the Platform or its servers are free of viruses or other harmful components.
11.3 No Warranty of Match Quality or Provider Availability
MatchMyDentist does not warrant that a match will be generated, that any particular Provider will be available or will accept a match, that any match will result in a successful appointment, treatment, or outcome, that any Provider’s qualifications, credentials, or standing are current or accurate, or that the matching algorithm will identify the most compatible Provider for any given Patient.
11.4 Assumption of Risk
You acknowledge that your use of the Platform is at your sole risk. You are solely responsible for evaluating the suitability of any Provider you are matched with and for all decisions regarding your dental care.
11.5 Jurisdictional Limitations
Some jurisdictions do not allow the exclusion of certain warranties. In such jurisdictions, the exclusions above apply to the fullest extent permitted by applicable law.
Limitation of Liability
12.1 No Indirect Damages
To the maximum extent permitted by applicable law, MatchMyDentist, its officers, directors, employees, agents, affiliates, licensors, service providers, and contractors shall not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, including, without limitation, damages for lost profits, lost revenue, loss of goodwill, loss of business opportunities, loss of data, business interruption, or other intangible losses, arising out of or relating to these Terms or your use of the Platform, regardless of the cause of action or theory of liability and even if MatchMyDentist has been advised of the possibility of such damages.
12.2 Liability Cap
To the maximum extent permitted by applicable law, the total aggregate liability of MatchMyDentist arising out of or relating to these Terms, the Platform, or the Services, regardless of the form of action or theory of liability, shall not exceed the greater of two hundred United States dollars (US $200) or the amount, if any, you paid directly to MatchMyDentist during the twelve (12) months immediately preceding the event giving rise to the claim.
This limitation does not apply to liability resulting from MatchMyDentist’s gross negligence, willful misconduct, or any liability that cannot be limited or excluded under applicable law, including certain statutory privacy rights where required by law.
12.3 No Responsibility for Treatment
MatchMyDentist is not responsible or liable for any diagnosis, examination, treatment recommendation, treatment, prescription, procedure, clinical decision, billing dispute, insurance determination, missed appointment, canceled appointment, unsatisfactory appointment, communication, disagreement, or dispute between a Patient and a Provider, or for any injury, illness, loss, damage, adverse outcome, or other consequence arising from or relating to dental care or the independent actions or omissions of any Provider.
12.4 Basis of the Bargain
You acknowledge and agree that MatchMyDentist has established its business model, pricing (if any), and these Terms in reliance upon the limitations of liability, disclaimers, and allocations of risk set forth herein, and that such provisions constitute an essential basis of the agreement between you and MatchMyDentist.
12.5 Jurisdictional Limitations
Some jurisdictions do not permit the exclusion or limitation of certain warranties, damages, or liabilities. In such jurisdictions, the limitations and exclusions contained in these Terms shall apply only to the fullest extent permitted by applicable law, and the remaining provisions shall continue in full force and effect.
Disclaimers & No Guarantees
13.1 No Medical or Dental Advice
All content, communications, and Services provided through the Platform are for informational and administrative purposes only. MatchMyDentist does not provide dental or medical advice, diagnoses, clinical assessments, treatment recommendations, emergency services, or healthcare services of any kind. Nothing on the Platform should be interpreted as professional dental or medical advice or relied upon as a substitute for consultation with a licensed healthcare provider. If you are experiencing a dental emergency, call 9-1-1 or seek immediate emergency dental or medical care.
13.2 No Guarantee of Provider Introduction, Match, Care, or Outcome
MatchMyDentist does not guarantee that a Patient Profile will receive a Provider introduction, that a Provider will choose to submit an introduction, that a Patient will accept a Provider’s introduction, that a Match will occur, that communications between a Patient and Provider will be successful, or that any appointment, treatment, clinical outcome, financial outcome, or patient experience will occur or meet your expectations.
13.3 Provider Credentialing — Scope and Limits
Before a Provider participates in the Platform, MatchMyDentist collects the Provider’s representations and supporting documentation regarding licensure, malpractice insurance, and professional and legal history, and confirms that the Provider’s dental license appears active through the applicable state dental board’s publicly available license-verification system.
MatchMyDentist does not independently verify or audit the accuracy or completeness of information provided by a Provider beyond confirming the Provider’s license status through the applicable state dental board’s public records. MatchMyDentist does not continuously monitor a Provider’s license, malpractice insurance, disciplinary history, professional standing, legal status, or other credentials, and does not conduct malpractice-history investigations, criminal-background investigations, or comprehensive credentialing reviews unless expressly stated otherwise.
A Provider’s participation in the Platform does not constitute or imply an endorsement, recommendation, certification, guarantee, or representation regarding the Provider’s qualifications, competence, conduct, quality of care, or current professional standing. Patients remain solely responsible for evaluating whether a Provider is appropriate for their individual needs and are encouraged to independently verify a Provider’s license, disciplinary history, and professional standing through the applicable state dental board or other appropriate regulatory authorities before accepting care.
Dispute Resolution & Arbitration
14.1 Mandatory Individual Arbitration
Except as provided below, any dispute arising out of or relating to these Terms or the Services will be resolved exclusively through binding individual arbitration, not in court, administered by JAMS under its rules, in Florida. Judgment may be entered in any court with proper jurisdiction.
14.2 Class Action Waiver
You waive any right to participate in a class action, class arbitration, or representative proceeding. All claims must be brought individually.
14.3 Preservation of Non-Waivable Rights
Nothing in this Section limits any right that cannot be waived under applicable law, including the right to seek public injunctive relief where such relief cannot be waived (e.g., under California law). To the extent a claim for public injunctive relief is found non-arbitrable, it may proceed in court while all other claims remain in arbitration.
14.4 Small Claims Exception
Either party may bring a qualifying individual claim in small claims court.
14.5 Right to Opt Out
You may opt out of this arbitration agreement by sending written notice to the address in Section 18, including your name and account information and a clear statement that you opt out, postmarked within 30 days of first agreeing to these Terms. Opting out does not affect any other part of these Terms.
14.6 Limitation Period
Any claim must be filed within one (1) year from the date the issue arises, except where a longer period is required by applicable law.
Termination
15.1 By You
You may stop using the Services at any time. To request deletion of your account and data, contact support@matchmydentist.com. Account deletion may be permanent.
15.2 By MatchMyDentist
We may suspend, deactivate, or terminate access at any time, including for violation of these Terms, prolonged inactivity, false information, disruptive behavior, legal or regulatory requirements, or discontinuation of the Services. We are not liable for loss of access or data after termination, subject to our data-retention policies.
Governing Law & Jurisdiction
These Terms, and any dispute arising out of or related to them, are governed by the laws of the State of Florida, without regard to its conflict-of-law principles. Subject to Section 14, the state and federal courts located in Palm Beach County, Florida shall have exclusive jurisdiction.
Force Majeure
MatchMyDentist will not be liable for any delay or failure to perform resulting from causes beyond our reasonable control (acts of God, natural disasters, war, civil unrest, labor disputes, supply-chain failures, government action, cyberattacks, power or internet outages). Such events suspend our obligations for the duration of the disruption.
Notices
MatchMyDentist may provide notices to you by email, through the Platform, by in-app message, by posting notices on the Platform, or by physical mail, where applicable.

MatchMyDentist Inc. is a Delaware corporation.
Entire Agreement & Modifications
These Terms, together with the Supplemental Policies, constitute the entire agreement between you and MatchMyDentist concerning the Platform. We may modify these Terms; material changes will be communicated by email, dashboard notice, or posted updates, and continued use after updates constitutes acceptance.
Severability & Waiver
If any provision is held invalid or unenforceable, the remainder remains in effect. Our failure to enforce any provision is not a waiver of our right to enforce it later.
Relationship of the Parties
Nothing contained in these Terms shall be construed to create any partnership, joint venture, agency, employment, fiduciary, franchise, brokerage, referral, or representative relationship between you and MatchMyDentist.
You may not represent yourself as acting on behalf of MatchMyDentist, hold yourself out as our agent or representative, or bind or attempt to bind MatchMyDentist in any manner without our prior express written authorization.
